Membership & Telehealth Terms

Effective Date: July 12, 2026

These Membership & Telehealth Terms (“Terms”) govern participation in membership programs and telehealth services offered by Synergy Health & Wellness LLC (“Synergy Health & Wellness,” “we,” “our,” or “us”).

By enrolling in a membership program, scheduling services, or participating in telehealth appointments, you acknowledge that you have read and agree to these Terms.

1. About Our Practice

Synergy Health & Wellness LLC is a cash-pay healthcare practice providing personalized wellness services including, but not limited to:

  • Hormone optimization

  • Medical weight management

  • Metabolic health

  • Preventive health

  • Lifestyle medicine

  • Nutrition counseling

  • Laboratory interpretation

  • IV therapy

  • Telehealth services (where permitted)

  • Other wellness services as determined appropriate by your provider

We do not bill commercial insurance, Medicare, or Medicaid for routine office visits unless otherwise stated.

Patients are responsible for all fees associated with services received.

2. Membership Programs

Synergy Health & Wellness offers optional membership programs designed to provide ongoing support and continuity of care.

Membership benefits may include services such as:

  • Follow-up visits

  • Laboratory review

  • Secure messaging

  • Care coordination

  • Treatment plan adjustments

  • Educational resources

  • Other services described in your selected membership

Specific membership benefits, pricing, and duration are described separately at the time of enrollment and may be updated periodically.

3. Membership Is Not Health Insurance

Memberships are not health insurance.

Membership fees do not constitute insurance premiums and should not be interpreted as providing insurance coverage for healthcare services.

Patients are encouraged to maintain appropriate health insurance coverage for services not provided by Synergy Health & Wellness.

4. Individualized Care

Recommendations are based on each patient’s:

  • Medical history

  • Symptoms

  • Laboratory findings

  • Medications

  • Goals

  • Clinical evaluation

Treatment recommendations vary from patient to patient.

Enrollment in a membership does not guarantee any specific treatment plan, prescription, laboratory result, or clinical outcome.

5. Initial Evaluation

Most patients begin care with an initial evaluation designed to review their health history, concerns, laboratory data, and goals.

Additional testing, laboratory work, or follow-up visits may be recommended before treatment decisions are made.

6. Laboratory Testing

Laboratory testing may be recommended as part of your care.

Patients are responsible for laboratory fees unless otherwise specified.

Outside laboratory results may be reviewed at the discretion of the provider.

Additional testing may be recommended if previous laboratory results are incomplete, outdated, or insufficient for clinical decision-making.

7. Prescription Decisions

All prescriptions are issued solely at the discretion of the treating provider based on medical necessity, applicable laws, clinical judgment, and patient safety.

Enrollment in a membership does not guarantee that any medication will be prescribed, refilled, or continued.

The provider reserves the right to decline, discontinue, or modify treatment when clinically appropriate.

8. Telehealth Services

Telehealth appointments are offered when clinically appropriate and permitted by applicable law.

Patients are responsible for:

  • Ensuring they are physically located in a state where the provider is authorized to practice at the time of the visit.

  • Having access to a reliable internet connection and appropriate technology.

  • Participating in the visit in a private setting whenever possible.

If technical issues prevent completion of a telehealth appointment, the appointment may need to be rescheduled.

9. Appropriate Use of Messaging

Secure messaging is intended for routine, non-urgent communication.

Examples include:

  • Medication questions

  • Follow-up questions

  • Laboratory coordination

  • Appointment scheduling

  • Administrative requests

Messaging should not be used for:

  • Medical emergencies

  • Urgent symptoms

  • Chest pain

  • Shortness of breath

  • Severe bleeding

  • Stroke symptoms

  • Suicidal thoughts

  • Any condition requiring immediate medical attention

Call 911 or go to the nearest emergency department if you are experiencing a medical emergency.

10. Missed Appointments & Cancellations

Appointment cancellation and no-show policies are communicated separately and may be updated from time to time.

Patients may be responsible for applicable fees for late cancellations or missed appointments.

11. Payment

Payment is due according to the policies in effect at the time services are provided.

Membership fees, consultation fees, laboratory fees, supplements, medications, and other services may be billed separately.

Failure to maintain payments may result in suspension or termination of membership benefits.

12. Membership Changes

Membership benefits, pricing, and available services may be modified periodically.

Existing members will be notified of material changes when required by law or consistent with applicable agreements.

13. Ending a Membership

Patients may request to end their membership according to the membership agreement in effect at the time of enrollment.

Any outstanding balances remain the responsibility of the patient.

Termination of membership does not affect financial obligations for services already provided.

14. Pharmacy & Laboratory Partners

Synergy Health & Wellness may coordinate care with independent pharmacies, compounding pharmacies, laboratories, imaging centers, and other healthcare providers.

These organizations operate independently and maintain their own policies, pricing, privacy practices, and terms of service.

Synergy Health & Wellness is not responsible for delays, pricing changes, product availability, shipping, or services provided by independent third parties.

15. No Guarantee of Results

Healthcare outcomes vary based on many factors, including medical history, genetics, adherence to treatment recommendations, nutrition, lifestyle, medications, and underlying conditions.

Synergy Health & Wellness makes no guarantee regarding:

  • Weight loss

  • Hormone optimization

  • Symptom improvement

  • Laboratory improvements

  • Disease prevention

  • Treatment success

16. Limitation of Liability

To the fullest extent permitted by law, Synergy Health & Wellness LLC shall not be liable for indirect, incidental, consequential, or special damages arising from participation in membership programs or telehealth services, except as otherwise required by applicable law.

Nothing in these Terms limits rights that cannot legally be waived.

17. Governing Law

These Terms shall be governed by the laws of the State of Utah.

Any disputes arising from these Terms shall be resolved in a court of competent jurisdiction located within the State of Utah.
