THE SHORT ANSWER

## Can You Have Thyroid Symptoms Even If Your TSH Is Normal?

Yes. You can have symptoms that feel like a thyroid problem even when your TSH is in the normal range.

TSH is a useful screening test, but it is one measurement, and the symptoms most people associate with the thyroid are not specific to it. A normal result means the thyroid is less likely to be the main driver, not that your symptoms are unreal.

Fatigue, weight change, brain fog, feeling cold, and hair thinning can also come from iron deficiency, insulin resistance, perimenopause or menopause, nutrient deficiencies, poor sleep, medications, or other metabolic changes. Some people also have thyroid antibodies, as in Hashimoto’s, while their TSH is still normal.

A normal TSH does not prove that something was missed, and it does not make routine testing useless. It means that when symptoms persist, a broader clinical evaluation can be reasonable.

DOES THIS SOUND FAMILIAR?

## When Something Feels Off Even Though the Numbers Look Fine

You were told your labs are normal, and you may have been relieved for about a day. Then you went back to a body that doesn’t feel like yours. You sleep and still wake up tired. You eat the way you always have, and the scale moves the wrong way.

Many people also describe feeling dismissed. That experience is common, and it is a fair reason to ask for a closer look. These symptoms have many possible causes, and more than one can be happening at the same time.

- Exhausted despite sleeping a full night
- Weight gain, or weight that will not come off
- Brain fog and trouble finding words
- Feeling cold when others are comfortable
- Hair thinning or more shedding than usual
- Constipation that has become routine
- Low motivation that is not like you
- Slow recovery after exercise
- New changes during perimenopause or menopause

LOOKING AT THE FULL PICTURE

## Thyroid Is Only One Piece of the Pattern

Synergy looks at thyroid function alongside the other factors that can produce the same symptoms. Testing is individualized. Not every patient needs every test, and we choose what to review based on your symptoms, history, prior results, and current treatment.

### Thyroid function

TSH, free T4, and free T3 read together with your symptoms. Thyroid antibodies, such as TPO, when appropriate to look for autoimmune thyroid disease like Hashimoto’s.

### Iron and nutrients

Iron and ferritin, vitamin B12, and vitamin D can relate to fatigue, hair changes, and feeling cold. We check them when your history suggests they matter.

### Metabolic markers

Glucose, insulin, and related markers help show whether insulin resistance or other metabolic changes are part of weight and energy complaints.

### Sex hormones

In women, the hormone changes of perimenopause and menopause can mimic thyroid symptoms, so they are considered when your age and history point that way.

### Sleep and medications

Poor sleep, sleep disruption, and some medications and supplements can affect energy, mood, weight, and lab results. We review them as part of the history.

### Your story

How your symptoms started, how they have changed, and what you have already tried often tells us more than any single number.

THYROID AND HORMONES

## Thyroid Symptoms and Perimenopause Can Look Surprisingly Similar

Perimenopause is the transition before menopause, when estrogen and progesterone begin to fluctuate. It often starts in the 40s, and sometimes earlier. The symptoms it causes overlap heavily with the symptoms of an underactive thyroid.

Thyroid conditions are also more common in women as they get older, so in midlife both can be present at once. That is why we look at hormone and thyroid patterns together, rather than assuming every symptom belongs to one or the other.

### Symptoms that can come from either

Fatigue and low energy

Weight changes

Sleep problems

Brain fog

Feeling cold, or temperature swings

Hair thinning or texture changes

Mood changes and low motivation

WHY SYNERGY

## Why Patients Come to Synergy After Being Told Everything Looks Fine

Synergy is not a generic hormone clinic or a broad longevity clinic. We focus on persistent thyroid, hormone, and metabolic symptoms when standard care has not fully explained why you still feel unwell. We work alongside conventional care, and we respect the testing you have already had.

### One clinician follows your case

You see the same experienced clinician from start to finish. No rotating providers and no handoffs.

### Patterns, not isolated numbers

A value that is technically in range can still mean something in context. We read results alongside your symptoms and history.

### Priorities, in order

When several things may be contributing, we work out what deserves attention first instead of changing everything at once.

### Only the testing that is useful

We do not order tests to look comprehensive. Each one should answer a question about your care.

“We don’t treat a lab value. We figure out what is most likely driving how you feel and what deserves attention first.”

HOW WE WORK

ONGOING SUPPORT

## You Won’t Be Left to Figure This Out Alone

Thyroid and hormone changes rarely resolve with one prescription and one set of labs. Your body responds over time, and treatment often needs to be adjusted as it does. Care at Synergy is built around that reality.

Ongoing management requires enrollment in a recommended care program. What is included depends on the program you select and your clinical needs.

Scheduled follow-up visits

Repeat labs when appropriate

Clinical guidance as your results come in

Support between visits within your program

Treatment adjustments based on how you respond

YOUR FIRST STEP

## Start With a Clarity Consultation

The $197 [Clarity Consultation](/clarity-consultation) is a 30-minute starting visit to review your symptoms, goals, health history, and recent labs and determine the appropriate next step.

YOUR CLINICIAN

## Cammi Fruin, AGNP-C, BHRT-C

Cammi is a board-certified nurse practitioner with 27+ years in clinical healthcare. Her background includes oncology and nephrology, and she focuses on overlapping thyroid, hormone, and metabolic concerns in adults.

“One of the patterns I see repeatedly in clinic is a patient who has been told her labs are normal and who is, quite reasonably, still unwell. Often the answer is not one dramatic finding. It is a few things stacking up, such as iron that is low-normal, sleep that has fallen apart, and hormones that are shifting. My job is to sort out which of those matters most and start there.”

Can thyroid symptoms occur with a normal TSH?

Yes. Symptoms such as fatigue, weight change, and brain fog can occur with a normal TSH. TSH is one screening measure, and these symptoms are not specific to the thyroid. They can also come from iron deficiency, sleep problems, hormonal changes, or metabolic factors. Hashimoto’s can also be present while TSH is still normal.

What thyroid labs does Synergy look at?

Testing is individualized. Depending on your symptoms and history, it may include TSH, free T4, free T3, and thyroid antibodies such as TPO and thyroglobulin. We interpret the results together with your symptoms rather than one number alone.

Do you treat Hashimoto’s?

Yes. Cammi sees adults who already have Hashimoto’s or hypothyroidism, including people who take thyroid medication. Ongoing management requires enrollment in a recommended care program. See our Hashimoto’s and thyroid care page.

Can perimenopause cause symptoms that feel like thyroid problems?

Yes. Perimenopause can cause fatigue, weight change, sleep problems, brain fog, temperature changes, hair changes, and mood shifts, all of which also appear with thyroid disease. Both can be present at the same time, so we evaluate them together.

Can low iron cause thyroid-like symptoms?

Yes. Low iron or low ferritin can contribute to fatigue, hair shedding, feeling cold, and poor exercise tolerance. We check iron markers when your history suggests they may be relevant, not for every patient.

Do I need to stop my thyroid medication before testing?

No. Do not stop or change your thyroid medication on your own. Take it as your prescriber directs and tell us when you take it relative to blood draws. Also tell us about supplements, including biotin, because some supplements can affect certain lab results.

Does Synergy prescribe levothyroxine?

Cammi manages thyroid medications when appropriate, based on your symptoms, labs, history, and response. Not every patient with symptoms needs thyroid medication, and a higher dose is not automatically the answer.

Does Synergy use desiccated thyroid medication?

Medication choices are individualized and made by Cammi after your evaluation. If you take, or are curious about, a specific thyroid medication, including desiccated thyroid, bring it up at your Clarity Consultation so it can be discussed in the context of your history.

Do you see patients from Park City?

Yes. Our office is in Heber City, and we serve patients from Midway, Park City, and the surrounding Wasatch County area.

What happens after the Clarity Consultation?

Cammi reviews what you shared and recommends the appropriate next step. When it is appropriate, that is the $670 Clarity Assessment, and your $197 is credited toward it if you complete it within 30 days. Ongoing management requires enrollment in a recommended care program.

Does Synergy accept insurance?

Please call the office at (435) 315-9960 to confirm current insurance and payment information before you book.

Can I use HSA or FSA funds?

Eligibility for HSA and FSA funds depends on your plan. Please call the office at (435) 315-9960 to confirm what applies to your visit, and check with your plan administrator.
